Our day services are suitable for adults over the age of 17 who have been diagnosed with a learning disability and/or autism spectrum condition, as well as mental health needs, challenging behaviour and associated complex needs. We’re also proud to support individuals with any of these conditions who are also blind, deaf or have profound and multiple learning disabilities.
Some of the people who attend our day services also live in an Achieve together supported living or residential care home; others use our Domiciliary support services to live an independent and fulfilled life in their community.
Day care services are provided as part of your care and support plan, which comes from your assessment with your social worker. We can help you apply for funding, so let us know if you need our support with this.
Registering to join our day services is an easy process. All you need to do is call our referrals hub on 03301 755 332 or email referrals@achievetogether.co.uk. Our Referrals and Assessments Managers will then explain everything you need to know, answer any questions you may have and advise what availability there is at that time – you can then decide which you’d like to join and we’ll arrange for your first session.
