This modern supported living home, Chatham, supports adults with learning disabilities and or autism spectrum conditions who are transitioning from residential college, foster care or the family home to a more independent lifestyle.

The support team are able to support people from a young age as part of a transitional model of support we promote at Achieve together. With each person supported, we work closely with all parties to ensure an agreed approach is put in place to help people make that transition on to adulthood a smooth and positive one.

The home promotes independence and supports young people to access further education, employment, social and recreational activities. Person-centred plans are developed for each person that supports them in becoming more independent in all areas of their lives.
